Registration is required for each child participating in Rampart PAL activities. Follow the steps below to complete the application and submit the required information.

    Print and completely fill out the form.

  • 3. Visit a Doctor

    Have your child’s doctor complete the appropriate section on the application to confirm medical clearance.

  • 4. Scan documents

    Scan the completed application and your child’s most recent report card into your computer.

  • 5. Upload

    Fill out the form below, upload your scanned documents, and submit the online form.
